Mensicus tear: Meniscus tears are usually caused by twisting injuries to the knee.   As we get older, then amount of energy needed to tear a meniscus decreases as the properties of the meniscus change.  Complex tears involve injuries in multiple planes of the meniscus rather than one single tear.  Older individuals will often have these types of tears and degeneration in the knee likely plays a role.

Poss surgery: The  bucket handle tear of the meniscus will likely cause you problems - should probably be dealt with via arthroscopic approach.   The ACL tear can wait - do PT and make a decision later if you want to have an ACL reconstruction done.  Not everyone needs ACL repair - talk to your doctor about the pros and cons.

Extrusion: Extrusion refers to the medial meniscus extending past the end of the tibia the bone that it rests on, radiographically on your MRI. Don't worry, your Orthopaedic Surgeon will help you with this. Please stay calm and see Ortho.

Meniscus: If symptomatic then the answer would be yes. Not all meniscus tears especially partial ones require surgery. However please understand that the meniscus has very poor healing capacity on its own.

Maybe: A finding of a tear on MRI does not mean you need surgery. The  key to determining if you need surgery is reproducible pain that prevents you from performing the activities that you enjoy. Swelling is another factor that would indicate that surgery is necessary.

3 months: Appropriately treated fractures typically heal within 3 months.  Meniscus tears seldomly heal on their own, but they may not cause symptoms.

Depends: Some degree of improving soreness or swelling after repair is normal; I would just make sure your surgeon is aware as it might also be a symptom if the repair has not yet healed.

A lot depends: on severity of your symptoms and progress since the injury.  Definitive surgical treatment is menisectomy.  You should discuss with orthopedic surgery specialist.

No: I would think that before you had surgery, you should have a new MRI, not one that was done almost a year ago.

Many ways: Twisting when knee flexed @ about 30 degrees, worse if u r struck from the side in this position. Twisting in a deep knee squat, again worse if struck while in this position. Also twisting while walking on uneven ground . There r more ways but these r common mechanism of injury.
