Do not worry: A bone spur on top of the patella is a sign of chronic inflammation of the quadriceps tendon and may be seen in patients that sustain a quadriceps tendon rupture--most often men in their 40s and 50s.  Most bone spurs rarely involve the joint and their presence alone does not warrant surgery.
With physical activity, proper warm up and a cool down period that includes gentle stretching can help.

Rest, ice, elevation, and compression of the area should help as well as taking something like Alieve or ibuprofen.

When cartilage is abnormal, this is called a chondral change, usually on an MRI or surgical report.  Chondral changes may include thinning of the cartilage, or loose and damaged areas of cartilage.

Hyaline cartilage is a highly specialized material that is 100x slicker than wet ice on wet ice!  hyaline cartilage is only about 5mm thick in most joints, and it can wear away as we age.
