DEPENDS: If the internal bleeding is intravisceral (in stomach or gut) there will probably no feelings until one becomes anemic enough to be tired or dizzy.  If the bleeding is intraabdominal, in the cavity of the abdomen, blood is an irritant, so there will be pain and discomfort.  Spenic bleeding will also cause left shoulder pain. Variable: Depends on the cause. Bleeding stomach ulcers may have no symptoms except for the visual clue such as vomiting blood or having dark black bowel movement. Same with colon bleeding; no symptoms except maybe seeing red blood from rectum. Otherwise major traumatic injury such as fall, car accident, knife or gunshot wound would cause pain & bleeding.

Painless: Internal bleeding can be painless.  Appendicitis almost always painful.  You are taking Ibuprofen so an ulcer although unusual at 18 years old, would also be painful.  Appendix pain usually starts around the belly button and settles in the right groin.  Fevers can also occur.  Bleeding is not common.  A meckels diverticulum could bleed.

Bleeding: If you have signs of internal bleeding, then go to the nearest ER and they will evaluate you.  They will perform blood testing, support your blood pressure with intravenous fluids and if necessary, call in a specialist to evaluate you further.

Many things: There are many causes of internal bleeding. Ulcers of the stomach/duodenum, major artery rupture, crohn's disease, small bowel tumors, colon cancer, colon infections, ulcerative colitis, vascular lesions of the colon,  liver cirrhosis with enlarged esophageal veins. Trauma from car accident, gun or knife injury. Symptoms may be none with mild anemia,  to severe weakness and low blood pressure.

Depends: There could be little symptoms such as mild weakness and anemia. Sometimes vomiting of blood or passing dark old blood in the bowel movements if there is intestinal bleeding. Sometimes there may be cramping abdominal discomfort. There may be no abdominal pain. However, if you mean bleeding from a severe accident with liver or spleen damage, then abdominal pain and distension would be noted.
