Painless: Internal bleeding can be painless. Appendicitis almost always painful. You are taking Ibuprofen so an ulcer although unusual at 18 years old, would also be painful. Appendix pain usually starts around the belly button and settles in the right groin. Fevers can also occur. Bleeding is not common. A meckels diverticulum could bleed.

Possibly: But this depends upon why your are bleeding, where and how much. If you are having abdominal pain, nausea and vomiting and fever and other symptoms of this nature go and see a doctor immediately.
