: There can be many reasons for this. Improper collection of urine (contamination), infection by organisms such as Mycoplasma. Your symptoms are consistent with UTI hence you are getting better by treatment prescribed for it

Bacteruria: Unless they put a tube up into her and catheterized her to get the urine sample, bacteria form the skin and genital area can get into the urine sample. This is especially true in little girls where there are lots of nooks and crannies where skin bacteria can hide. They may see these bacteria when look at the urine under microscope, but they don't grow in culture in just skin source bacteria.

Full evaluation: You need to have a full urologic evaluation. This should include upper tract imaging such as ultrasound or ct scan as well as cystoscopy and vaginal exam.

: Almost by rule, doctor should discuss with patient about the condition and work-up result(s), how to treat and how to prevent the recurrence in the future if applicable. Because chlamydia is a STD so your doctor definitely has to inform and treat you and your sex partner should be treated as well.

Probably not: Typical care for pregnancy includes a urine culture in early pregnancy.  If that was negative and you have no symptoms, I wouldnt' treat at this point.  Your obgyn can be more specifically helpful at your next visit.

May be a : Problem in collection/transport (bactericidal agents in the container?, delayed transport to the lab? - urine specimens need to be processed within 2 h of collection). A selected fastidious bacterial agent may not grow in usual culture media.

Depends upon type.: In most instances, urine should not contain any bacteria, and the presence of bacteria might indicate an infection.  Sometimes when a urine culture is collected, a few bacteria from the skin or end of the urethra may "contaminate" the urine.  In such cases, bacteria may not indicate a real disease.  Need to know type and # of bacteria and whether there are accompanying white blood cells in urine.

Yes: All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Having said that, your urine culture result does not indicate an infection.
