New guidelines: Sounds like you're doing all the right things with a  healthy diet and staying at a healthy weight.  New guidelines were released last year, where we are no longer trying to reach a specific target number but rather treating patients who have certain risk factors.  Talk to your doctor about these risk factors.

Slightly: We like for total cholesterol be be less than 200 so that is somewhat high. However, your good cholesterol the HDL is high and this is good, over 60 is excellent. The LDL should be less than 130 unless you are diabetic or have heart disease in which case it should be less than 100. Triglycerides are normal. If you have diabetes or heart disease you probably need medicine, otherwise diet/exercise.

Hi ldl. Low hdl: Your LDL should ideally be lower, but falls in range for initial dietary change and exercise therapy. Regular exercise can also help hdl, but may take several months to show a benefit. If you smoke, this needs to be stopped- suppresses HDL and damages he inner wall of your blood vessels, predisposing to depositing cholesterol in arteries.

is tranzadone better than remeron (mirtazapine) to reduce my trigl level of 413? i am on tricor 200mg.

Antidepressants: Both trazodone and Remeron (mirtazapine) are antidepressants, and aren't prescribed to work on triglycerides. Remeron (mirtazapine) especially may increase your appetite -- so this warrants consideration if you're concerned about being overweight. Both it and trazodone are somewhat sedating and might help with sleep disturbance also.

what is my target ldl?
replace niacin w/statins?

Exercise and MD: You should consult your doctor for the advisablity of going on a statin, as you will need to be monitored. Also seek your doctor's clearance for engaging in increasingly vigorous exercise for at least 30 minutes a day.

: All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Talk to the doctor who ordered the tests. More than lipids: All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Talk to the doctor who ordered the tests. Having said that, the single most important issue is your weight and you need to reduce it through diet and exercise. Reducing weight will address lipids and blood pressure.
