C-section: Unfortunately, a transverse position in pregnancy can be a dangerous situation.  Once labor starts, typically the lowest part of the baby to enter the birth canal is the baby's shoulder.  Hundreds of years ago, this situation would be fatal complication, because the baby could never fit through the birth canal...But today, a safe and timely c-section would prevent injury to both mom and baby.

C-section: Babies may remain transverse for a variety of reasons. Sometimes there is a benign growth in the uterine wall or other issue that prevents movement into a normal position. Regardless of the cause, these are only deliverable surgically.

Baby can change/not: If position is transverse the OB may try a maneuver called a "version" to externally shift the baby into a more favorable position for natural delivery. If that  does not work you will need a c-section.

Likely to improve: placenta previa diagnosed in second trimester most often resolves by 3rd trimester. Usually by that time, the fetus is head down and vaginal delivery is an option. 
Follow closely with ultrasounds this pregnancy to help guide your decision. Unlikely but if the placenta previa persists, then a csection is needed

No: If the first twin is head down then you can try for a vaginal delivery.  They can try to turn the second baby as the first one is coming out so that the second baby is head down.  Sometimes this doesn't work and you have a small chance of needing a c-section for the second twin.

Will likely flip!: At 25 weeks, babies in utero are moving all over the place. Generally, they line themselves up in the last four weeks or so - into a head-down presentation. I've seen a few situations where babies have actually flipped from head-up to head-down during labor! there are variations in the way a specific uterus is made that will encourage a baby to stay in a transverse lie. Ask your doctor!

Dont worry, most babies change position before Delivery.
Try to tilt your pelvis forward, rather than back, when you're sitting. Check that your knees are always lower than your hips. This is called optimal fetal positioning (OFP) and it encourages your baby into an anterior position. It changes your posture, particularly when u are sitting down. Good Luck:).

: No. When a fetus assumes the sideways position, fundal measurements derived from studies of head down babies do not apply. You would expect the sideways position to reduce fundal height.

Being prepared: Being prepared helps a lot in challenging situations. This applies to a situation, when there is transverse presentation with placenta previa and a planned c/section. In this particular situation, ultrasound is a great tool to get prepared and minimize the risk.

Fetal position.: At 26 wks gestational, it is not unusual to see a transverse position. It is prudent to get an obstetric ultrasound to confirm placental location and rule out fibroid.
Most of the transverse presentation at this time will rotate to vertex close to term. You can use Tylenol which is safe in pregnancy for comfort and inform your doctor accordingly. Good luck.
