No: If the first twin is head down then you can try for a vaginal delivery. They can try to turn the second baby as the first one is coming out so that the second baby is head down. Sometimes this doesn't work and you have a small chance of needing a c-section for the second twin.

Most likely: It depends how many weeks gestation you are now. Since the malpositioned twin might still rotate to a vertex position close to a term age. On the other hand, the twins are rarely get to the term gestation.
