Not typical : This does not sound like a typical hiatal hernia. Without any disregard to the skills of your nurse, it sounds as if you need advanced diagnostic imaging plus a possible upper endoscopy of upper GI study.

Tightness c/w muscle: contraction, which (if enough &amp; sustained) can become painful. All our muscles (heart muscle more so; especially inner walls nearest chambers) ↓ blood flow during contraction. The contraction pressure opposes &amp; ↓es blood flow into muscles via arteries &amp; capillaries while ↑es flow outward into the veins. Thus sustained contractions induces ischemia &amp; (if long enough) contractions become painful.

Tearing chest pain: Tearing, tight chest pain under underneath left rib cage is serious problem. It may be heart, aorta, or pleuritis. Please go to er and have ct scan of chest to rule out above conditions, specially aortic dissection. Thanks.

Many causes: Tightness in the chest area can be a sign of a heart attack.  If this persists, then you may want to go see a doctor asap.   Urgent care or er.   Other causes could be muscle soreness, gastrointestinal issues or lung issues.  Please have your md evaluate you.

Vague symptoms: could be muscle spasm but so hard to tell without more info. good there is no pain. if bloating, it could be constipation, gas, IBS, GERD, tumor (very unlikely).

Exam and X-rays : Some people have a degree of kyphoscoliosis which can make the spine appear to slant to one side.  You health provider can determine if you have this condition.   Many cases are mild and do not require any specific treatment.

Back pains: If the discomfort under the rib cage is in the right upper front abdomen, gall bladder disease is a probable cause. With mid or left upper abdominal discomfort, pancreatic pathology is a possibility.
. Pain in the back and flank is characteristic of a kidney problem.

A physician would evaluate you and order appropriate tests.

Needs further eval!!: At your age, need to know duration/ onset of symptoms, need physical exam!! Our concern is missing a tumor or malignancy!!Labs and imaging with CT or at least sonogram, symptoms could mean Gallbladder problems- gallstones? What other symptoms? Weight loss? Urine changes? Bowel changes? Food makes symptoms worse??! Best if seen by internal medicine, GI or surgeon! Good Luck

Doctor can evaluate: A person with chest pain symptoms usually has to go to her doctor (or even to the emergency room) to be checked right away for a heart problems, UNLESS there is another convincing reason for chest pain (decided upon by talking with one's doctor). That means not waiting at home to see if it turns into a more serious condition. As the saying goes, "better safe than sorry".

Diaphragm mass: that mass is called your xyphoid process (small bone just distal to your sternum - totally normal finding) - flutters probably small muscle spasm - go see doctor eventually to confirm what I said
