No but,,: A low INR is not a direct cause if pain.  It stands for International  Normalized Ratio and is the ratio of the patients prothrombin time to a normal prothrombin time value raised to the power of the ISI (international Sensitivity Index) or a ratio of tissue factor to a reference tissue factor.  However if it's low it can mean that the blood is clotting more and can lead to thrombosis in patients.

Maybe: Doctors often examine for increased pain with certain movements to help determine if a deep vein thrombosis is present; however these tests are not very accurate.

Yes, as for the DVT: You need to have it confirmed with a doppler us, then be started on blood thinners.
Seeing you pcp will be helpful to start this and he/she can treat the shin splints, which is much less serious problem, than the dvt. If any chest symptoms (pain, shortness of breath, coughing, etc) occur, you need to go a hospital, for controlling the pe, a much more serious condition.

See an orthopod: After making sure that no deep vein thrombosis in that leg, you need to see an orthopedic surgeon, mentioned you had knee pain, inflammation may be affecting the ankle as well. As for breastfeeding, pain killers like paracetamol or NSAIDs as ibuprofen won't affect your baby , but tell your doctor if s/he wants to prescribe something different, good luck

That is a good: Question as sometimes it is clinically difficult for us too the physicians. To be sure one can do a blood test called d-dimer test but the best and with immediate results is an ultrasound duplex test. That will immediately show if clot is there or not. Trauma can cause clots and muscle pain. So, for a sudden calf pain one has to see a doctor asap as a clot may need specific attention.

See below: Thrombosis of the radial artery can occur from trauma, but if have had not thromboembolic workup to see if you have a clotting disorder, you may want to consider this.

Superficial or deep?: I assume you are asking about a venous thrombosis (vt).  There are two kinds, superficial (svt) and deep (dvt).  An svt can be more symptomatic than a dvt.  With svts, people usually have pain, warmth, redness, and swelling over the affected segment of vein, or at least part of it.  Classically, the symptoms of DVT include swelling and pain, but often there are no symptoms at all.

Ultrasound: Dvt or deep vein thrombosis is often diagnoses with an ultrasound of the affected limb.  If the clot is suspected to be in the lung then a ct scan can be used.  These scans can also be used to scan the legs as well.  If you are concerned about having a DVT then you need to be seen by your pcp for further evaluation.

It depends: For deep vein thrombosis systemic anti coagulation is best. The mainstay is Heparin followed by coumadin (warfarin). However their are several newer options. The newer options have advantages and disadvantages.  
For phlebitis/ thrombosis in a superficial vein the treatment depends on the size  location of the thrombosis/phlebitis systemic anti-coagulation is used or sometimes less aggressive treatments used.

could i have a thrombosis with no symptom?

Thrombosis: Depending on where the thrombosis is, etc, you may need treatment to prevent a more serious condition such as a pe, mi, or stroke, etc.  Best for you to discuss with your doctor what treatment you need, if any, depending on the severity and location of your thrombosis (even without symptoms).
