It depends: For deep vein thrombosis systemic anti coagulation is best. The mainstay is Heparin followed by coumadin (warfarin). However their are several newer options. The newer options have advantages and disadvantages. For phlebitis/ thrombosis in a superficial vein the treatment depends on the size location of the thrombosis/phlebitis systemic anti-coagulation is used or sometimes less aggressive treatments used.

Anticoagulation: Deep venous thrombosis requires anticoagulation to prevent extension of the clot and possible pulmonary embolism.
