It should help: The ferrous form of iron is usually the best absorbed.  Some people tolerate ferrous fumarate or gluconate better than ferrous sulfate.  GI side effects are usually the problem.  I presume your ferritin level is 4 ng/ml, which is low suggesting iron deficiency and is the likely cause of feeling poorly.  Hopefully you can tolerate the ferrous fumarate and resupply your body's iron.

No...: You should definitely consult your doctor and make sure that you have low iron stores in your body. If you have normal iron levels and you take too much iron it can accumulate in your liver and cause damage to it. Make sure you get seen by your doctor and have blood work done in order to find out if you need supplement if you are iron deficient.

Typical dose: The dose you describe is rather standard. Some advise 325mg ferrous sulfate three times a day.
I usually advise only 325mg once a day unless anemia is quite severe as higher doses tend to cause constipation.

Yes: All these forms of iron have distinct characteristics that affect these areas. Though gluconate and fumarate are more tolerable than the sulfate, they provide a lower amount of absorbable iron. I feel that it is more important to be able to tolerate the form of iron you're taking than how much is absorbed especially if you have any stomach problems to begin with.

: Iron level is not a good marker for iron deficiency by itself. Ferritin reflects your body's iron but many conditions increase the ferritin level even you may be relatively deficient in iron. TIBC should be done to know if you truly have low iron. By those # I'd say you may have low iron but not 100% sure and without Hb/Hct/MCV/MCH/TIBC, I could not say if you should take iron supplement or not.

: It would be prudent to see a gynecologist to address the heavy periods. If hemoglobin is normal, intravenous iron administration may not be warranted, it should be kept as an alternative in case oral iron does not help.
For healthy diet: http://www.choosemyplate.gov/., exercise 30 minutes/day, drink enough water daily so your urine is mostly colorless, have safe sex, no tobacco alcohol or drugs.

Iron supplements: Iron sulfate is the supplement of choice for iron deficiency, but iron fumarate has similar bioavailability and CAN also be taken for iron deficiency. Since any iron can interfere with absorption of levofloxacin, make sure your take any iron at least 2 hours before or after the levofloxacin dose to avoid this problem.
