Allergy not just GI: Dairy intolerance is caused by an inability to digest milk sugar lactose into two simpler sugars. This results in gas, cramps, bloating, flatulence & diarrhea. Dairy allergy may cause these same symptoms but typically also causes vomiting, skin rashes like hives or eczema & even triggers allergic rhinitis & asthma. If symptoms all GI try lactose-free milk. You'll have no symptoms if intolerant.

Milk intolerance: While this can happen it is hard to prove. The usually reported symptoms are fussiness after breast feeding, perhaps somewhat like colic, diarrhea, possibly vomiting (unusual). The test you can do is to eliminate cow's milk for a week and see if it makes a difference (but you will need to supplement the calories, vitamins, fats and proteins with something else).

Lactose vs Milk: Some individuals have a Lactose intolerance in which there is lactose free milk and supplements and then others have a true mild allergy.Therefor Milk Allergy is not Lactose intolerance so depends on which you are or what you can supplement.

Best to have it : Checked out . Hard to guess what those spots are without seeing them. How long has this been going on  and they would appear exactly a day or so after he has the milk? Do they go away by itself? Does it look like hives? Have you documented those spots? Do take pictures so you can show them to the doctor.

Analysis: Your diet has likely very little to do with whether B-12 levels stay normal, as this depends on the gastric function of intrinsic factor, and if impaired via any mechanism will lead to decrease absorption.  Symptoms can and do occur at 280, but certainly not as severe as if lower.  You likely would benefit from injectable B-12 shots.  Contact your doctor.

Yes, : Appendicitis can be confused with almost any disease, and its manifestations are considered "protean".  I had to look up this word in the dictionary when i first heard of it, too.  If you suspect acute appendicitis, you need to be seen by a doctor immediately.

Symptoms and test: Dairy reactions can be lactose iintolerance (milk sugar) with symptoms limited to GI tract like crampy abdominal pain, gassiness and diarrhea.  Milk skin test is negative.  Milk allergy (to milk protein) may have GI symptoms but also rash, hives, swelling, or even anaphylaxis.  Milk skin test will be positive.

Lactase-treated…: Milk (lactaid, dairy ease) or lactose-free cheese, yogurt, ice cream should present no problems. Using lactase enzyme tablets (lactaid, dairy ease) before dairy products will break down milk sugar that causes problems. Enjoy your dairy products as good sources of calcium.
