Sunitinib Dosing: Sunitinib is usually given for 4 weeks, then 2 weeks off, and the response (reduction in tumor burden) assessed.  Often, the dose is reduced if side effects supervene.  There is no "standard" protocol and only your oncologist and the rest of your treatment team can recommend the best protocol for you.

Other drugs worked: Nexavar, torisel, and sutent all received FDA approval because they improved progression free survival.

The initial Trovax trial was terminated early because there was little or no prospect of demonstrating a statistically significant survival benefit compared to placebo.

See your doctor: Congestive heart failure is usually treated with diuretics and other medications tailored to your situation. The advice of your internist oncologist, or cardiologist with respect to treatment would be best in this situation.

Several.: This would best require a visit with a cardiologist, to review all his meds.  It seems that beta blockers and diuretics would be ok with this protein kinase inhibitor.

Sunitinib: This drug may be associated with severe/fatal liver toxicity. It may produce chf, heart attack, irregular heart rhythms, blood clots, hemorrhage, low blood cell counts (neutrophils & thrombocytes). However the benefits against renal ca may well outweigh the risks. Your oncologist will no doubt monitor liver and other functions very closely to prevent complications.

It is a good drug: Sunitinib is a  good drug which is very powerful. So you should start it soon and hope that it works(it works in less than half of the patients(40-50%). Another new treatment approved recently is immunotherapy with Nivolumab(Opdivo). This is a powerful drug to keep in mind and use it either before Sunitinib or as next therapy in case Sunitinib failed to benefit you.

D/w oncology team: If he doesn't respond to sunitinib then if he wishes for more therapy he should move on to the next line therapy. Axitinib- is something he should consider- as it is more tolerable than sunitinib.Other option also include- temsirolimus iv, affinitor, bev/ifn or palliative care. He also need a good pain control obviously. If elligible, a trial using stereotactic radiation and il-2 is available.
