Need surg consult: A Grade 2 Spondylolisthesis definitely needs to be evaluated and treated by an orthopaedic or neurosurgeon who specializes in spinal surgery.  You are just on the border of requiring spinal surgery, i.e. a fusion, with a Grade 2 slip.

The inguinal pain may/may not be related to the back. Time to see your surgeon for an up-to-date evaluation

Inguinal ligament: Maybe the ligament itself is injured?  But there is no pain with palpation?  It is not likely to be from your lower back spondylosis.  That would be more likely to affect your feet and calves.

x-ray on lumbar spine with flex ext says -bilateral pars defect at l5 with approximately 3 mm anterolisthesis relative to s1, spina bifida occulta at l5. my mri shows the same as well as- mild facet arthropathy at l3-4 and l4-5. what's this mean?

: It means you need to look at the x-rays with your Dr and have them explain exactly what is going on. Different Drs will used differnt terms for the same thing.  eBe certain you have told him all the symptoms you are having. Your back is talking to you it seems. Listen and take good notes, share with your Drs and you all will figure it out. .

i got this feedback from an x-ray. there is lumberization of s1 with spina bifida occulata there is anterior wedging of the t12 body mild ddd at l5s1. should i be concerned?

Lumbarization: of S1 is an anatomic variation, usually of no concern. Likewise, spina bifida occulta is a minor variant, usually an incidental finding of no concern.  Mild "ddd" is mild degenerative arthritic changes at that level. T12 anterior wedging could also be an anatomic variation, or indicate prior compression fracture, would need to evaluate the xrays to try to determine which.

Combination of issue: Some of the back issues are congenital, some due to injury or wear and tear.  None seems surgical, and your symptoms can respond to conservative therapy.

? Connection: It would be good to know what your MRI showed and when it was done.  Since the pain affects these different areas, and it is transient, it is hard to know if there is a connection for certain, but it would be good to know what is going on there.

It was right to do the palpation exam- it may not be related to the spine after all.

Probably not much: Spina bifida oculta is often an x-ray finding that causes no major symptoms . However spina bifida can cause symptoms such as weakness of the legs and bladder problems and occasionally a characteristic loss of the sensation in the so called 'saddle' area.
