Hard to say: Difficult to determine why you have a high pulse rate when waking, but i would recommend that you see a dr. And get a complete physical to head off additional issues in the future.

No distractions: It may be from the fact that it is quiet at night and that there is nothing else for the brain to focus on. It could be that her blood pressure is higher lying down than sitting up. The carotid artery sits close to the eardrum, so the force of blood through it may cause a sense of a "heard heartbeat".

husband's pulse ox during sleep is 89% and he wears a c-pap. is this normal? i noticed on exhale he was causing the bed to shake.

Frequently?: If his oxygen saturation is below 91% frequently during the night he may need a higher setting. A repeat study or home study may be useful in making the adjustments.

Many causes....: There are many different problems that can cause a low pulse ox during sleep. A sleep test can diagnose obstructive sleep apnea as well as central sleep apnea. If neither of those are present as the cause of the low pulse ox, a pulmonary function test can determine if the airways are obstructed or restricted. An eval by a lung doc can help make the diagnosis and treat the cause.

One of the less : common things that can cause it is pheochromocytoma, a usually benign tumor of the adrenal glands. If you had a family history of pheochromocytosis, you'd probably know about it. Certainly tell your doctor about the episode.

Pulsation: You may be feeling the pulsation of arteries in the ear if the pulsation is synchronous with your pulse which can be palpated with your index finger placed just in front of your ear. It is recommended that you have your physician check your blood pressure to make sure it's not elevated and also to confirm what you are feeling. Your symptom may be more of a nuisance rather than anything serious.

is it safe to take one 25mg hydroxyzine for panic anxiety four hours after one drink? i'm also on daily bubroprion and sertraline, but can't sleep because of racing pulse (at rest, for no apparent reason)?

: Not feasible  to be precise, but advise a full medical re-evaluation, especially the rapid pulse, which may be indicative of a cardiac tachy-arrhythmia, and certainly thyroid functions should  be measured.  Would have your psychiatrist get involved as this proceeds.  In view of your described complexities, might consider local medical school second opinion, as an alternative.
