Yes - but very rare: B"sd do you mean small cell lung cancer? Elevated cortisol can be related to stress or to pituitary, adrenal or rarely, ectopic ACTH as in lung cancer.  The best screening test is late night salivary cortisol (or a blood cortisol - but this is inconvenient in late PM).  Normal cortisol is highest in the AM and close to zero late in the day.  This is complicated. Glad to discuss.

Mgt: The antibody may be associated with lung cancer. I would suggest having samples obtained from sputum and/or lung lavage. Three samples may be sent for Cytologic analysis. This should be further discussed with your physician since there are other details of your history that are missing. Risk factors such as smoking history and family history should be considered.

Sclc: Combination of Cisplatin and Etoposide is the first line therapy for small cell lung cancer. It is also one of regimens we use for non small cell lung cancer. Thus, this regimen will take care both. It is important however to make sure that your oncologist get more information including type ( adeno vs squamous etc),  some molecular evaluations ( egfr, alk, ross-1)  to the nsclc on the right lung.

is the primary tumor in sclc hard to visualize on imaging? if so does this mean it is cancer of unknown primary if you can find it only in lymph node

See below: The primary tumor for small cell lung cancer is usually easy to visualize on CT scan.  If it is SCLC, that is the primary type.  Often, the cancer develops in lymph nodes and if there is no primary tumor, only enlarged nodes, this is likely the primary site.

Not generally: Small cell lung cancer (SCLC), while generally a serious diagnosis, wasn't thought to affect the eyes. (The bones &amp; brain are still the most common sites of metastasis (spread)). Eye metastasis is STILL VERY RARE in SCLC, but cases have been described in medical literature. Normally it causes VISION LOSS NOT BLURRING as it affects the choroid, the part of the eye w/ most blood vessels. TTY Oncolog

Ask his doctor: Lung cancer frequently spreads to the brain. This patient might already have it in the brain(the fact he is getting radiation being given now...? Which part of the body). Brain involvemnt by lung cancer can cause loss of limb function. Which should not be a surprise.

Zero: I'm sorry about your cancer. Small-cell grows very fast, usually from nothing, and folks typically remember symptoms beginning days or a few weeks before diagnosis. The cough, weight loss, and malaise that may herald this illness are also those of emphysema from the smoking that usually causes small-cell. I hope you get effective treatment and with today's Rx perhaps great luck.

Very unlikely: I'm uncertain of the significance of the positive anti-VGKC test but I suspect it can be positive in the absence of a neoplasm. The likelihood of a nodule being malignant can be stratified by your age and the size of the nodule. LIKELIHOOD OF CA BY AGE: 35-39=3%, 40-49=15% ,50-59=43%,  &gt;60= 50%. LIKELIHOOD OF CA BY SIZE: &lt;5mm =-&lt;1%, 5-9mm =2-6%, 8-20mm =18%, &gt;20mm =&gt;50% chance of CA
