Wow, : Wow, this is a tough question. Naltrexone is an opiate antagonists used to decrease cravings for alcohol and opiods in people who abuse them. Obsessive compulsive disorder is not an approved indication for use of  naltrexone. With that said, I have heard anecdotal reports of Naltrexone being tried at doses of  25-50 mg per day for certain types of obssesive / compulsive behaviors.  This is called off - label use.   I have not used Naltrexone for this purpose, so I do not know how beneficial it might be. Maybe there is another psychiatrist on this site who is experienced in using Naltrexone this way.

: Sure, start with your doc who has been treating you as he or she has your medical records and can have a thorough discussion with you about goals. Such treatment should be followed tightly and treatment involves a team including psychologists, counselors and may involve group meetings.

: Most drugs in children are prescribed based on weight or body surface area. Perhaps he is a "big" boy and 50 is OK. Discuss with his Drs and caregivers.   He may be taking other meds that affect how is body metabolizes the 50 mg. Go to Drugs.com and look up this and enter all meds he is taking into their drug interaction checked. Then show to caregiver and ask them to share with his Dr.

: Because Naltrexone is used off label in children, there are not enough good studies to indicate appropriate levels of monitoring. Depending on the childs age, diagnoses, and other medical conditions, during the initial titrating period, careful monitoring should focus on liver enzymes and kidney function. The prescribing Dr will reccomend a specific regimen of testing to follow these parameters.
