An abscess: A lump on the gums is an abscess.  Typically, there are 3 reasons for it. 1) endodontic - the pulp of the tooth is infected.  Could be due to a cavity or trauma. 2) periodontic - the is an infection around the tooth. There is a space between the tooth and gums where bacteria can get trapped and abscess. 3) a fractured tooth. 
An abscess is treated with antibiotics and removal of the infection.

Swollen Gingiva: Your best bet is to see a dentist. The ER will most likely just prescribe antibiotics which will not solve the issue. Call a dentist for an emergency appointment.

Most likely they will need to be removed, but that is based upon the angle of the teeth and whether or not there is room enough in your mouth for them to erupt and serve a function if they do. but chronic gum disease is correlated with heart disease
