Keep it clean: After 5 days if the pain isn't significant, gentle rinsing with salt water to keep food out of the socket is often a good idea. Once the bone covers over, it will become more comfortable. Eventually the healing will be full and normal but it takes longer after a dry socket. Of course, going back to the treating dentist is a good idea for further evaluation but home care is important.

TMD: I do not believe that the extraction of your premolar CAUSED your TMD. You most likely had this previously to some extent and the procedure precipitated an acute episode. Soft diet, anti-inflammatory medication, warm hot compresses to the outside of your face just in front of your ears and an evaluation by a dentist who is knowledgeable and experienced in treating patients with TMD is advised.

Premolar: The extraction of a premolar should not cause any TMJ issues... you most probably developed the problem due to other causes. An examination by a local dentist with knowledge regarding TMJ causes and treatment will shed some light on how to treat your condition. All the best...

Extraction or not: It depends.  If the bone of the upper arch of teeth are too far forward then extraction might be necessary.  In most cases extraction is not needed.  If the upper lip looks correct then removing upper teeth is contraindicated.  
If the front teeth are visible before you walk around the corner then it might be indicated.
The orthodontist should be using the cephlometric xray analysis to decide.

Depends: It really depends on the severity of the protrusion. If the amount of retraction requires more than a few millimeters of space your orthodontist may consider extraction.

Dental care: There are always alternative treatment choices, although they may not be as good.  Maintaining tooth root with root canal treatment and crown usually better than extraction/implant/crown.  Extraction alone is not advisable.  Talk to your Dentist about payment plans.  Seek treatment at a clinic or Dental School.  But best choice is probably to work with yourDentist.

Probably: Implantation is usually about a week after ovulation, so meds will not affect the fertilized egg.

can tooth extraction gap on the side (second bicuspid) at least be reduced without braces?

: You may want o consider a dental implant and crown to replace the tooth entirely. To move the two molars behind that space towards the mesial is a tall order even with orthodontics.
