Nerve damage: Post-infectious gastroparesis (pigp) is a subgroup of idiopathic gastroparesis. This is usually caused by virus. Symptoms may take  4 weeks to over 12 months to resolve.It is believed that the virus somehow  damages the nerves going to stomach.

30 minutes (or more): You need to find out how long it takes your body to start to absorb the medication -- usually that's about 30 minutes, or more, depending on your degree of gastric emptying.  If your gastric emptying is 0-15% at 2 hours, consider taking it an hour before; 15-30% - 45 minutes, and 30%+ 30 minutes should work out well.  Good luck!

Unlikely: For gastroparesis to cause fever. The cause for gastroparesis, viral or autoimmune may cause also low grade fever. Drug fever is also common in case you started new drugs.

: Never use abbreviations. I cannot answer as I am not certain what igf1 stands for where you are located. Immunoglobulin Funky 1 or what?  This can be deadly in medicine. I would look for cause of gastroparesis first and treat that.
