IBS ? : 40&#x27;s female post-hysterectomy has &quot;sharp shooting crampy pains in pelvis&quot;. Assuming chronic &amp; intermittent, then Irritable Bowel Syndrome, IBS, possible. See a Gynecologist &amp; if no specific findings consider IBS, wherein the pain generator is intestinal spasm. A second opinion might be provided by a Gastroenterologist. See comments below:

Yes: A pap test looks for cervical cancer. If you had a full hysterectomy, it means you don't have a cervix and therefore can't get cervical cancer and you don't need a pap test.  You will however need a pelvic exam, becuase that checks our pelvis and ovaries, which you will still have after a hysterectomy.

Further monitoring a: Further monitoring is advised. This should include repeat CA 125 testing twice more , done 1-2 months apart. Imaging of the abdomen and pelvis with US is a good idea. If the CA 125 continues to increase progressively then I would consider further interventions. Otherwise it may be just a fluke.

Yes: How remote from hysterectomy, why do you have inflmmation? Nsaids in general are very good antainflammatory medications and should help. But most important is to define what is the problem at hand.

Resee your doctor.: It is unclear as to why you are having pain. Either resee the same doctor or have a consultation with another GYN for a second opinion.

when should a hysterectomy be done for pelvic pain?

Multiple indications.: If 1) pain is chronic and interfering with normal activities, sexual functioning, mood, and/or relationships, 2)conservative measures like birth control pills have failed, and 3) no future fertility is desired, then a hysterectomy can be considered. For some diseases, like endometriosis, a hysterectomy is the only definitive cure.

No age: It is common for women to believe that a pap smear is the only reason for a pelvic exam and that can stop once a hysterectomy is done. The cervix and uterus are not the only things evaluated during a pelvic exam. Specifically after hysterectomy, i'm more inclined to evaluate for loss of pelvic support and look for visual abnormalities.

Colon?  Bladder?: Could be a colon or bladder problem, such as infection, kidney stone, constipation etc.  you will need to be checked by a doctor to see what the problem is. Good luck!

Pelvic pain: There are many causes of pelvic pain.
You need more help from your doctor to determine cause of pain and treatment.   If you are not getting the help you need, you may consider a consultation with another doctor, preferably a gynecologist.
