what causes a loose filling? i had a loose filling fall out that was only two years old. does that mean the dentist did something wrong? was there something wrong with the filling or adhesive?

Many : Many factors can cause loose fillings.  In order to determine the reason, a radiograph and clinical exam would be necessary.

Oh yes.: Decay leads to infection; can get bigger, lead to pus pocket (abscess) in and around tooth. Can become a big problem if it is not addressed-usually incision and drainage, maybe root canal, maybe more advanced oral surgery; requires antibiotic treatment.

Biting Pain: Sounds like your filling as been comfortable until recently. Sudden biting pain is usually related to a cracked tooth. The happens a lot with large fillings and someone with a strong bite. See your dentist. At the least you probably need a crown, if the crack extends into the nerve you may need a root canal. If the tooth is actually split, you could loose the tooth. See your dentist soon.

Dental Pain: You've asked multiple times regarding your tooth sensitivity. Atypical implies out of the ordinary. The tooth is sensitive due to dentinal fibrils being damaged. The fillings can be removed and the tubules sealed with a sealing agent. This should help if not eradicate your sensitivity. Shallow restorations usually do not result in sensitivity.

Yes: You are never too young to need a filling.  If  the tooth has decay, the longer you wait, the more problems you can have.

Advances in modern dental technology now allow the to place tooth colored composite fillings instead of amalgam fillings to restore the natural appearance of the tooth.

Hard to say...: This type of question needs to be evaluated with a radiograph and in person, something that is not possible over the internet. You'll need to have each of your treating dentists explain themselves.

Severity: Old filling can be revel and replaced.  But if tooth severely compromised there may not be enough tooth structure left to adequately support a filling for a long period of time.  If not, best solution would be to crown the tooth to support the remaining tooth material.  Seek the care that will give you the longest lasting result.  Not being able to examine tooth, can't make choice for you.
