Yes: If you are 46 years old, your age may be an issue. Rh factor disparity between you and your husband will not get in the way of your getting pregnant. It would be prudent to consult your OB even before you try to get pregnant so that you are on proper antenatal care.

Depends: For a true crossmatch, possibly. But males are often given 0+ blood due to lack of availability of adequate 0-, which is reserved for females of childbearing (/pre-childbearing) ages. We always need donors to keep the blood supply supplied. Call 1-800-red-cross to find a donor center near you!

Probably unrelated: Rhogam is used in rh - patients who may carry an rh +  fetus to prevent isoimmunization. It's to prevent antibodies against rh + fetuses and prevent an hydropic fetus (lethal condition). Her miscarriages may be unrelated but you're correct, usually rh - women receive Rhogam within 72 hours of possible exposure to rh + blood (as in miscarriage). Prenatal labs always check for these abnl antibodies.

Count your calories.: It has been shown that the only effective weight loss strategy is reduced caloric intake. Exercise plays a minimal role at best. Of course this will be more difficult for you with the diabetes however close glucose monitoring with reduced caloric intake should be doable. See if you can establish with a nutritionist to help you.

Father's blood type: Father is O positive.   In the A-B-O nomenclature, A and B and dominant and Rh + is dominant over Rh -.  That means that the baby had to get an O allele from both mother and father and at least one Rh + allele from a parent.  Mom is OO neg neg and that means that the father has to be OO pos neg or OO pos pos.   Either way, the father is phenotypically O pos.

A little overkill: There is absolutely no harm in getting the shot.  However the chance that this tiny bit of bleeding represents a risk that the baby's possible Rh positive blood could possibly get into your system sufficiently to sensitize you is really, really remote.  I would consider it only if you actually miscarried.

Unlikely: This is an unlikely cause. However, there are blood tests the doc can run on you to see if you have been sensitized in any way to the rh factor due to that earlier event.It is a test that is often performed early in pregnancy, so it may have been part of your 2nd pregnancy workup but never discussed (if it was negative).

A few: Being o negative makes you the "universal donor" this means that very few people would react to your blood and would make you a very popular donor at the blood bank.   In pregnancy, if the fetus has a different blood type, it adds an increased risk of blood type incompatibility with the fetus- although this is easily remedied by your OB through the use of Rhogam vaccine during pregnancy.
