A little overkill: There is absolutely no harm in getting the shot. However the chance that this tiny bit of bleeding represents a risk that the baby's possible Rh positive blood could possibly get into your system sufficiently to sensitize you is really, really remote. I would consider it only if you actually miscarried.

No correct answer: There isn't a lot of evidence to support Rhogam with first trimester spotting. It is probably unnecessary, but due to an overabundance of caution, most doctors and hospitals administer it with any signs of spotting or bleeding at any time in pregnancy. That doesn't mean it's necessary. If you have red bleeding, I'd get the shot for sure.
