Unlikely : Based on your description, I would not think this would be enough force to cause a concussion. Concussion symptoms include headache, dizziness, light and noise sensitivity, memory deficits, blurred vision, etc. If any of these develop, then seek medical treatment. Fortunately, isolated concussions are not usually emergencies.

"s.O.M.R" stands for "source oriented medical record" - with specific sections: history ; physical, nursing notes, progress, laboratory and diagnostic testing.  Each section is completed by individuals responsible for those sections and usually do not cross-reference data from one section to another.

Need TBI specialist: Your narrative is common. The spectrum of Mild TBI and Post concussion syndrome is often lacking coordination of care differently than severe TBI.

Migraine Treatment: There are many good medications for migraine treatment. Migraine is a serious and complex neurologic disorder. Today effective management of migraine involves three specific strategies: 1) patient education 2) non-pharmacological management (diet, sleep hygiene, acupuncture, MRI and 3) pharmacological (triptans, topamax, (topiramate) etc) See headache or orofacial pain specialist for treatment.

i broke my head open a year ago that needed just to be glued back together and then another time 8 months ago. since then i have noticed changes in my emotions and mental state. do you think the change in my emotions is due to the head injurys?

: Head trauma such as you experienced can have associated secondary inflammatory reactions that can affect the brain significantly. I recommend that you have an evaluation by a neurologist which would include cognitive testing as well as possible referral for psychiatric help if deemed advisable after your assessment.

It : It is possible but unlikely, that the head injury has anything to do with infertility.  The best test is for him to check his sperm count.  This really shouldn't be very expensive but would answer a lot of your questions.  Best wishes.

Emergency Room: If you consider the injury to be severe, lost consciousness or have a severe headache, go to the Emergency Room where they can  evaluate you and depending on the magnitude of the injury and symptoms do a Head CT scan.

Too vague: This is an incredibly vague ??? If you are concerned you can call your local ER and speak to an RN. If it seems more urgent, go to the ER. Hard to say much more than that given the wide spectrum of injury possible.
