Yes: Proton Pump Inhibitors such as esomeprazole (Nexium) are absorbed best and provide the best benefit when taken first thing in the morning 1 hour before any food.

why are you suppose to take nexium (esomeprazole) in the morning and an hour before a meal? why can't you take it with a meal?

Won't stop pumping : Meal intake will cause certain cells in your stomach to start pumping acid. Taking Nexium (esomeprazole) with meals won't give your body enough time to absorb it. Once the pump starts its hard to make it stop (inhibit) it which defeats the purpose of proton pump inhibitors. It may still work, but not as effectively. Taking it 2 hours after meals near bedtime is another option if am dosing is inconvenient.

Reflux: laying down after a meal will not change how the medication works. However, your reflux will be worse as food and acid will go up into your esophagus. This is independent of the medication. In other words, you need to use your medicine and not lay down after a meal for best benefit.

PPI: PPI is long acting and needs to be taken regularly to prevent acid formation. So you should take it regularly about the same time every day preferably in the morning.

Yes, but...: The nexium will still work but ideally it should be taken first thing in the morning before food. I would say if you want to take it in the evening, then take it about an hour before your evening meal and make sure to take it every day. Hope this helps.

Endoscopy: Did you have endoscopy, without egd it is impossible to determine the cause of your problem.  See gastroenterologist.

Wait: Iron should not be taken within 30 to 45 minutes after a milk feed, as the calcium decreases iron absorption.  No problem with vitamin d.

Diabetes ?: I presume you have been diagnosed with diabetes.  A blood sugar reading before meal time will tell your physician (and you) how well your blood sugar is being controlled. Right after a meal your blood sugar will go up normally and is not a good guide.  
Your physician may request a 2 to 4 hour after meal blood sugar too to see how well your body regulates sugar after a meal.
