Be seen: The concern at 5.5 months out is that you should be well healed from your surgery, and so you should not be having much pain at this point.  A ct scan can be done to determine if you repair was successful and is healed or if it was not successful which could explain your pain.  Let your surgeon know you are still hurting.

i have been suffering from painful knots in my upper right back for some time, they move around and when it pains my whole side is affected and lame. i have been suffering for years and only ever got tablets for muscle relief. help?

Maybe Fibromyalgia: You may have Fibromyalgia with trigger points, stress reduction and aerobic exercise would help, you should discuss this condition with your primary care provider and may be see a rheumatologist, I hope this helps

Muscles: I wouldn't recommend drugs to build muscle.  Just make sure you are eating well and taking in enough protein for your body to build those muscles.  There are protein supplements available as well.

Trigger point inj: The actually injection just allow the muscle to relax a little for you to do stretches and help prevent the muscle from being tense again. You can have them at least once a month if there is not steroid or Botox being used in it.

Tight shoulder: Adequate warmup, stretch before. Adequate cooldown, stretch after a workout or exercise. Ice followed by heat.Occasional topical creams.

Yes!: There are many resources that physicians & pharmacists rely upon regarding drug information. I like www.Epocrates.Com & www.Drugs.Com. While there are drug-drug interactions w/isotretinoin, there doesn't appear to be any nutritional issues (http://www.Drugs.Com/accutane.Html). Create Activity Plan: Do you have muscle weakness?  The symptoms suggest you need a complete evaluation because although these symptoms may be natural deconditioning  or minor strains, your doctor can assess you for other causes of myalgia (such as polymyalgia rheumatica).  After this assessment, your doctor can monitor your progress in a tailored exercise regimen that you should start slowly and advance over time.

Trapezius pain: May be muscle spasm from carrying things on one side more than the other. Try alternating arms. Use moist heat to neck & shoulders. Have friend/partner/massage therapist massage neck & shoulders. Do range-of-motion stretching exercises for neck & shoulders. If no improvement, see doc.
