: An orthopedic surgeon might choose a cortisone injection, or even hyaluronic acid.  Physical therapy might be an initial approach.  Would not anticipate need for surgery based upon your description.  Discuss at length with an orthopedist who specializes in knees.

Discuss with MD: In those few patients who do not respond to a non-operative treatment program, an arthroscopic resection of their medial plica may be indicated.

Therapy: A course of physical therapy and possible steroid injection is the initial approach to a stiff knee post surgery.  If you fail surgery then a knee arthroscopy with removal of scar tissue and adhesions should be performed.

Varies: It varies a lot.  Some patients may have a plica and do fine, not even knowing they have it.  Others have persistent pain, swelling, and popping in the knee and ultimately have surgery.  Meds, PT, ice, and possibly injection therapy are the typical nonoperative treatments to varying degrees of success.

Surgery is an option: Plica syndrome is common and can be quite a problem. Therapy is a reasonable start. If therapy and NSAID haven't helped, an injection would be the next best option. If that fails to relieve your pain, an arthroscopic surgery is reasonable.

A cause of knee pain: A knee "plica" refers to a band of fibrous tissue in the knee joint.  Plica are normally found in the knee and are usually asymptomatic.  Occasionally, a plica may become thicken and therefore cause knee pain.

If not willing to-: accept the risks, you need do nothing, but your symptoms will not get better. repeated steroid injections will cause damage to the joint surfaces. it's a great drug, but a double edged sword, beware!

A fold of tissue: Knee medial patellar plica is part of the lining of the knee joint. Normal plica are small. It can be injured and become scarred and large. Large plica can be pinched by your kneecap.  This is painful and is located on the inner side of your kneecap. Sports can aggravate it. An orthopedic surgeon can evaluate this and treat it with exercise, a sleeve, nsaids, and occasionally, arthroscopy.

Synovial fold: Medial plica is a thickening or fold in the synovial lining of the knee.  (plica synovialis mediopatellaris = medial plica: extends from the infrapatellar fat pad to the medial capsule of the knee, in the coronal plane.)  this is a common finding which generally causes no problems.  Very rarely it can cause a painful plica syndrome.

Depends: The severity of your knee injury that caused the tear of the medial patellar retinaculum tear will determine the length of your recovery.  Physical therapy can be very beneficial to aid your recovery and speed your return to "normal" activities. If you have not seen an orthopaedic surgeon yet, it would probably be worthwhile for you.
