Chicken vs. egg: Good question. Both depression and fatigue are common symptoms of D deficiency. I know of no data on which is more common, but you're correct that depression often causes fatigue, but fatigue and other symptoms of low D such as chronic pain can cause depression. No: Low vitamin D has not been linked as a cause of  POTS syndrome, but low levels could make it(POTS) much more difficult to manage. L,ow vitamin D can also contribute to fatigue, even without POTS.  If you decide to take vitamin D3, you should also take calcium supplements

Supplements: Low Vit D can cause fatigue.  Adequate sodium is necessary for your body to work properly..  as is iodine (added to table salt, to maintain thyroid health).  May need supplements to correct, follow up with a knowledgeable Dr for re-eval.

Perhaps: I have seen low vitamin D cause fatigue and slow thinking. Usually it's at a level even lower than 20 however anyone with the level of 20 should either take vitamin D3 supplements or get out in the sun. Drinking milk does not help your body's vitamin D level. That said,, my patients with POTS have other reasons for fatigue in general .

Unlikely: While vitamin D level of 21 is lower than the optimum, it is not likely to be the cause of your symptoms. You may take 2000 units of vitamin D3 daily,
For good health - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex.

Low Vitamin D: lowers immunity.  Fatigue and other symptoms can result and vulnerabilities from other diagnoses can become worse.  Raising Vitamin D3 is easy.  Speak w/your doc about taking 5K mg caps of Vitamin D3 daily and retesting in several months.  Peace and good health.

Microhematuria : Not sure of connection among these conditions.  But when there is blood in the urine(Hematuria), we must check for infection, stone disease, renal diseases and urinary cancer.  I would recommend seeing a urologist for cystoscopy

IFG: Hi. You have impaired fasting glucose (IFG) but mount a brisk insulin response to a caloric load. You're early in the type 2 diabetes process, probably in the "prediabetes" stage (you ARE prediabetic by IFG). Healthy diet &amp; exercise should be your lifelong lifestyle. There are medicines that help prevent progression to type 2 diabetes as well. Talk with your doc. Good luck!
