Scars: Start with topical silicone sheeting you can get on the internet to help flatten scar. Consider injections of kenalog (triamcinolone) to flatten if scare is thickened, and topical agents like neocutis biogel help as well. I offer laser treatments for my patients too. The earlier you treat a scar, the better the results

Revision surgery: Frequently cleft lip scars need to be revised to get them just right.  It should not be a major process but it can make a big difference.

I would encourage : You not to attempt self- treatments in this situation. Seeing a dermatologist is a safer option.

Not much: Unfortunately, most scar treatments offer only marginal improvement over doing nothing at all.  The biggest problem with the area around the lips is the amount of dynamic motion and tension that is created on the wound, which lends to a problematic scar.  I find the biggest factor is the amount of surgical debridement of injured tissue affects the outcome of the scar.

Not likely: This is called Chelitis and is usually caused from Riboflavin deficiency.  Are you Vegan ?  Mouth heals beautifully because of wonderful blood supply.  try Vaseline at night before bedtime to lips.  Drooling can cause an autodigestion and this will waterproof against this as well.

Proper Hydration: is very important. But large, painful pimples would be a good reason for dermatologic evaluation. It is doubtful that hydration alone will correct this problem.

Get tested: If you think you may have been exposed to HIV don't guess.  Get tested.  It is important to deal with this before it becomes advanced.  There are many causes of the symptoms you described.  If you both developed the same symptoms at the same time then the likely answer is an infection or chemical/toxin exposure.
