my 16 year old son has platelet count 255 and mean platelet volume 11.0 with a comment large platelets present.  what could be the cause of that?

Lab data: Without a clinical history and exam, lab data seldom has significance.  This may mean nothing at all.  Discuss with the doctor who ordered the testing.  Sometimes this simply means an "active" marrow or one which is being stimulated by some ongoing process, infection, other inflammation, etc, .

Nothing: It is of no clinical significance.  Just reporting incidental findings of normal variants.

Doubt it: Isolated, this little fragment of a complete blood count means nothing. You also have to know the idiosyncracies of the technician -- are there overcalls? It wouldn't surprise me. The basic numbers are probably more helpful. Best wishes.

i have low wbc 3.7, monocytes 22% (never been this high) and large platelets present. all else normal. i have lupus, ra,  factor 5 leiden. thoughts?

These findings: are pretty "classical" for early VIRAL INFECTION!
This can vary from Flu to Measles etc etc

Clotting problem: her hb. and platelets are normal
but her PT PTT needs to be checked and all the clotting factors should be checked so the bleeding due to clotting  problem should be checked
also local exam  and cauterize the bleeding vessel
in the mean time saline nose drops and a humidifyer might help

This make no sense: Please send us your CBC results. There is nothing like a platelet to large cell ratio!!. Give us some more info as to why you had this test in the first place and if you have doctor who can address this issue... it does not make any sense to me,.

: Large platelets on peripheral blood smear may indicate a disease process when taken in the right context with other lab results and signs/symptoms. When the platelet count is also low, it could be related to a heightened bone marrow response, associated with fragmentation of red blood cells, or just platelets that clump together because the blood was collected in a certain type of tube.

is this a normal fluctuation for platelets? 9/14/21-210 x 10 ^ 3ul, 10/8/21- 213 x 10^3ul, 10/14/21- 211 x 10 ^ 3ul, 11/5/21- 196 x 10 ^ 3ul. it seems like a big drop. i had pharyngitis could this cause the drop?

: This will depend on time of day, how long you have been out of bed and lab used. I always figure a 10% variation of most labs done on the same day from the same tube as usual variation.  Thus 10% of 210 is 21 and 210-21=189 so that looks pretty good just on one day but as it was over almost 2 months would consider range of variation OK.   But check with local lab as they may have varying variation

Vitamin & leukemias.: Hypersegmented neutrophils are seen with vitamin b-12 and folate (folic acid) deficiency, while large platelets can be in myelophthisic disease, like myelofibrosis and leukemias.  These are conditions that displace the precursor cells from the bone marrow into the peripheral blood.
