Large platelets on peripheral blood smear may indicate a disease process when taken in the right context with other lab results and signs/symptoms. When the platelet count is also low, it could be related to a heightened bone marrow response, associated with fragmentation of red blood cells, or just platelets that clump together because the blood was collected in a certain type of tube.
