If this is football tryouts or something similar, you have to consider the costs of extreme muscle development in a child that is not developmentally ready for this. You can have stress fractures and damage to muscle and joints. If this is to look "buff" for the girls then he needs to reorganize his priorities.

Both: But you have to consider that the fat loss and inches lost won't be replaced by bodybuilder type musculature.  It builds more muscle fibers and stonger bodies, so the answer is both.  But that level of cardio prevents people from packing on size.

Burn the fat: What tissue do you think is burning the fat?  It's the muscle.  If you increase your muscle, you will increase your fat-burning ability.  Don't be afraid of muscle.  It's good for you.  And you'll still look like a woman.

No.: Insanity does not contribute to muscle growth.  A well design exercise regimen, proper diet and rest do.

Lean body: The best lean body without muscle building is to eat a proper amd nutritional diet and do aerobic exercises. You will tone down and be nice and healthy.

Typical with plyo: Plyometrics/insanity puts a lot of stress on your knee joints that you don't get with other exercises. I recommend running on dirt/sand-it takes more energy and is gentler on your knees. It is also common for women to have arthritis under their kneecaps due to wider hips. You may benefit from getting a gait analysis at a good shoe store since pronation (either over or under) can cause knee pain.

Gain/lose muscle: Although this is a rather complex issue, my recommendation is to use your workout to either build mass OR increase cardiovascular ability. Heavy weights/low reps tend to build muscle. Low weight/high reps tend to burn calories and help endurance. You're likely to find that trying to do both in one workout will leave you achieving neither goal, feeling exhausted and frustrated with lack of progress

Weight loss: You should start by going on line to see "Body Mass Index" (BMI).
It will help you record your height and weight and let you know if your BMI is OK, too high, or too low.

Once you have that data you can begin to make adjustments:

Normal data: Relax and enjoy your workouts
Too low: Increase your food intake
Too high: Workout but also decrease your food intake.

Athletic Diet: Nope. Not at all.  You need to know two things.  You need more calories if this exercise regimen is real.  You also need to eat enough protein to keep up with the protein breaking down when your muscles exercise at this intensity.

Body building: Muscle building requires resistance training. Most body builders hypertrophy type i endurance fibers requiring medium weights multiple sets and a lot of reps combined with good diet.  Consider body for life, p90x or adonis golden ratio routines for a whole body program for strength, mass, power and weight maintenance.
