Occlusion: For young age (your baby), recession usually due to malocclusion, i.e., improper tooth alighment intra and/or interarch relationship.  Minor tooth movement can be done with your dentist help to alleviate this problem.  Check also if you are using soft bristle brush for brushing, not the hard or medium hard.  Brushing with soft toothbrush prevents  localized recession (usually occur in adult).

Tooth eruption: Primary (baby teeth) are almost always replaced with permanent teeth. However, sometimes the permanent tooth is either congenitally missing or developing at an angle that prevents it from erupting into its proper place in the mouth. Only through dental x-rays can one know what will happen in your particular case.

It : It may be that genetically this tooth bud formed in ths area of his mouth and is erupting a bit differently than the others.

He's in his growing years and still very, very young for you to worry about his permanent teeth. The permanent canine (eye tooth) erupts at about 11 years, +/-, so you get my point.

Its not that your dentist can do anything about that baby tooth right now. The tooth may naturally fall into place by itself.

Bleeding gums: Generally its such a slow process and your mouth heals so fast you dont notice it.  If the teeth are placky you can get bleeding.

Next tooth..: Very little in your body happens together - your body is not, contrary to popular belief, symmetric. I doubt his teeth popped through on the same day when he was a baby, and it's unlikely incoming adult teeth will follow an identical timeline. If you are having routine dental care for your child, your dentist will notice abnormal eruption, so i wouldn't worry about it and see dentist regularly.

Perhaps it's time: To have those baby teeth removed. The information on your post says that you are 48 years old. If that's true and those are your baby teeth, their days are numbered.  Regardless, you will have to see a dentist who can actually examine you, tell you what is going on, and suggest treatment alternatives.

I : I am sorry to hear about your troubles.  It is very unfortunate that you require so much work.  Your best bet is to contact a local dental school. Ask to be seen at the department of prosthodontics.  It is the specialty dedicated to the reconstruction and rehabilitation of the mouth.  You will be treated by the top dental school graduates who are learning to be specialists in this field.
Dental schools will also have a sliding fee scale or possibly pro bono.

Gently: If the tooth is loose enough for you to gently wiggle it out, you will not damage your orthodontic wire or brackets. If you can't do it, see your dentist.
