Throw out the shoes: The shoes are too small for you and causing trauma.  Neosporin or another ointment will help heal the nails, but the biggest factor is prevention by getting shoes that fit.  Shoes should allow wiggle room for toes and a thumb's width of length from the LONGEST toe.

Heel laceration: The real issue is how deep is the cut. If its very deep then it needs to be cleaned out and stitched. I would suggest that you show your doctor the site. Also bleeding should have stopped - the fact that it is still bleeding means its a deep cut and this should be checked.

Very very minor: The situation you described carries very very minor risk of contracting HIV. Typical testing scenarios involve a blood test for nucleic acids at 15 days, which is then repeated at 4 months. No test will show a positive result immediately after presumed exposure as it takes time for the virus to grow.
