Depends on dfn: Although both practices could be considered abusive in some form, by convention the withholding of food would fit the conventional use of the term. The term "enabler " would be used specifically to describe anyone who assists one with their self destructive activities.

Weight loss: Correct!!! Weight loss is about creating a calorie defecit (burn more than you take in). the average human can burn 1600-2000 cal /day  a bit more with exercise (20min on treadmill about 150 cal) . If you take in 1000 cal the deficit is 600-1000 cal. The problem is living on 1000 cal/ day is almost impossible.

Lose weight: Losing weight will probably improve your gynecomastia. You still might need surgery if it persists after losing weight. There is really no medicine to improve it. If you smoke marijuana or use steroids, stop as these promote gynecomastia.

You probably have: pulmonary hypertension from Pickwickian syndrome (obesity hypoventilation syndrome) which would result in chest discomfort and ventricular irritability (skipped beats.) You need to seek professional help for assisted weight loss or you will severely compromise your life expectancy. Good luck!

can emphysema be stopped if it hasn't progressed too far? air trapping and hyperinflation (rv/tlc 44%) but normal fev1/fvc. someone said those may seem normal due to severe obesity, however i thought that was accounted for in calculations?

: These values are part of a PFT report that has other results. An imaging study is often helpful. The RV/TLC ratio is elevated. The FEV1/FVC ratio is normal. Are you experiencing symptoms?  COPD is usually associated with a low FEV1/FVC ratio.

can being obese cause an enlarged spleen?  i was severely overweight, have fatty liver, but lft's are normal. so the liver seems to be functioning fine, wondering what could cause the spleen issue?

Yes: Non alcoholic fatty liver can cause portal hypertension which is a cause of spleen enlargement. Portal hypertension means that there is increased pressure and abnormal blood flow in portal vein system. Portal vein brings blood to the liver.

Usually not: There are few inherited conditions which cause obesity. Most obesity is not inherited, but obesity does tend to be "familial". One can be genetically destined to have more efficient energy use, smaller energy needs, or "slower metabolism"; and thus may become obese even if eating just moderate amounts of food; but he won't get obese if he wills himself to eat less food. He didn't inherit obesity.
