Possibly: Clomid (clomiphene) generally is used in women that do not ovulate on a regular basis.  And yes, it should cause a follicle to grow in size and should cause spontaneous ovulation, or the release of an egg, when the follicle gets to 22+mm in size.  If it is much bigger than that, Clomid (clomiphene) may have caused a simple cyst to develop and not an egg follicle.  Hope this helps.

Dosage of Clomid (clomiphene): The lowest dose that achieves is the dose to use.  I recommend that patients start at 50 mg/day for 5 days beginning on cycle day 3 or 5 depending upon whether they have a 28 or 30 days cycle.  If they don't ovulate on the 50 mg dose I increase it to 100 mg.

Depends: If you just skipped one day of the Clomid (clomiphene) then the growth would continue. If it was longer than that then the growth of the follicle or multiple follicles will have been compromised.

Did not ovulate: It sounds like you did not ovulate and the follicle is too small for d15. I suspect the Clomid (clomiphene) did not work. Your physician needs to decide what to do next.

Neither: The earlier you start Clomid (clomiphene) the more likely you will make make more than one egg. And also it makes for a more regularly timed cycle. But as far as overall effect it does not make any difference. The eggs are the same.

Need a diagnosis: The first "next" thing is to have a firm diagnosis or diagnoses. If you are missing any elements of a basic fertility eval you need to complete it. Sounds like you are saying you don't get a dominant follicle on CC? Or don't ovulate? Depending on your diagnosis you might boost the CC with metformin or dexamethasone, or may be a candidate for gonadotropin treatment. Good wishes.
