Mgt: I would expect some improvement in 48 hours, and the medication should be continued as directed for maximal benefit. Post viral inflammation may last weeks. If you spike a fever, please see your PCP to determine whether you have pneumonia. Are you being treated for influenza with Tamiflu? Did you have the flu vaccine?

: The drugs in drops/creams/lotions you apply to outside surfaces including your ear canal do not enter the blood. To reach the fetus and cause harm they must enter the blood, usually in things you eat/drink or have injected. I would not worry about these drops.

24-48 hrs to begin: I usually expect most antibiotics to begin to take hold within 24-48 hrs. That doesn't mean "cure, " but rather noticeable improvement. Be sure to complete the full course of antibiotics. Not doing so increases the risk of drug resistance - bacteria change to become resistant to the drugs! if your child is clearly getting worse, or if you're just not sure, call your doctor. : Second time you have asked -- previously you didn't say the site and I assumed sinus. But the answer is the same for the ear, probably otitis media. No antibiotic starts to improve symptoms (for any infection) as soon as 24 hr, and worsening pain for a couple of days also is common. Contact your doctor if pain is still increasing after another day or two, or if no improvement after 4-5 days.

Wait or change Rx: Studies have found that most ear infections will resolve spontaneously  by 6 weeks, but if causing severe pain or fever, or accompanied by sinusitis, then change in antibiotic would be indicated.  Both H.flu and M.cat bacteria may not respond to Rx with Biaxin, (clarithromycin) and switch to high dose amoxicillin-clavulanate.  if allergic to amox, (but not hives) use cefpdoxime or cefuroxime

Prescription: You really should be very careful about prescription medications that weren't prescribed specifically for your son  your son should really be evaluated and prescribed a medication specifically for him.  Particularly for children, medicines are prescribed by weight.  It also may not be the most appropriate drug for his condition.  He really should be evaluated and a prescription given to him.
