24-48 hrs to begin: I usually expect most antibiotics to begin to take hold within 24-48 hrs. That doesn't mean "cure, " but rather noticeable improvement. Be sure to complete the full course of antibiotics. Not doing so increases the risk of drug resistance - bacteria change to become resistant to the drugs! if your child is clearly getting worse, or if you're just not sure, call your doctor. Lgromkomd.

Depends : The med is a little slower to see response than your typical Amoxicillin or Augmentin (amoxicillin and clavulanate) as it works in a different method. Usually by the 3rd day the fever is down ( if it was up) & studies show the middle ear fluid rendered sterile by this point if the germ is susceptible. Fluid may persist behind the ear drum for a few weeks. Fever >3d is either med failure or other issue producing the fever.
