Sitting: Your pain is typical of someone who has degenerated disks.  There are many treatment options.  Start with physical therapy and nsaids... There are many new treatments for your pain.  Lumbar fusion surgery would be the final step if nothing else helps.  Se your healthcare professional to see what options are available for you and good luck.

Blood : Blood pressure medications often include beta blockers like metoprolol or atenolol.  These drugs can prevent one's heart from going faster when one exercises.  If your sister is taking one of these nmedications it might explain the difference in your heart rates.  
The effectiveness of aerobic exercise is often judged by the heart rate one achieves during the exercise.  For vigorous exercise, a target heart rate of 110 to 133 would be reasonable for an average 63 year old.  Consider discussing your target range with an exercise physiologist or personal trainer.  If you get short of breath or have chest tightness on the treadmill, you should definitely discuss this with your doctor.
Other medications which can lower your maximum heart rate include medications to reduce irregular heart beats, some calcium blockers and digoxin.  H-2 blockers prescribed for acid reflux may also reduce the maximum heart rate. Ask your doctor if you need to use a lower target heart rate.

Shin splints?: Is the pain predominantly over the inner shin and somewhat relieved after stopping?  Shin splints are more common with higher impact running, but can occur with intense walking program as well, especially with inadequate footwear. Pain is from inflammation of connective tissue sheath around muscle.  Usually improves with rest (continue exercise that limits stress in this area) and otc analgesic

Cardio: Your question is missing any kind of data to make a good answer.  The biggest issue is are you getting your heart rate to a level that is 'excercise'...Like at your age you should be at 130 to 140 beats per minute. If you are at this level you are exercising appropriately. If not you are just sauntering and wasting your time.

It may in terms as : To your posture and the carrying position of your head and neck. Try adjusting your elevation if feasible while walking or keep your head up and looking forward if you can do this safely on the treadmill.

Maybe: Depends on how many calories you eat vs. How many you burn on treadmill and what your basal calorie burn rate is. Also, the more muscle you have the bigger the "furnace" to burn those calories. Therefore, do wt training as well.

Yes!: You bet!  Just don’t make your goals too big - 40 minutes a day might be a lot - maybe start with 3x/week?  Or 20 min daily?  Along with your plan to cut down on calories!  Be sure to check with your doctor with any new symptoms after starting exercise - especially with any chest pain, difficulty breathing, dizziness, etc.

Good shape?: Normally you sweat as a way to cool down your body temperature.
Maybe your treadmill workout is too easy for you.
Maybe you need to increase your speed, elevation, etc to push yourself a little more. 
I think you'll probably sweat a little more?

Instep pain: Check the shoes and make sure that the lacing system is not too tight. When you are on the treadmill, your dorsiflexors are firing, possibly causing a tendinitis on the dorsum of the foot.Improving your heel strike, and spending less time on the ball of the foot may help.If you have a high instep, a prescription orthtoic can help prevent the instep from irritating.
