: Yes, your rheumatologist will be able to discern  the possible changes from steroids from changes truly induced by an autoimmune disease. There are options other than steroids once a condition is clearly identified.

Fog busters: Disorders of autoimmunity are commonly associated with cognitive dysfunction, so-called "brain fog", which can be steroid responsive. If you also have headaches, you may have cerebrovascular inflammation, for which tests are available. Cognitive functioning can improve with biological treatment used to arrest connective tissue disorders such as RA & are better than long-term steroid treatment.

Within 24 hours: While Prednisone and other oral steroids are rapidly absorbed from the GI tract and begin to kill eosinophils in the blood (common white blood cell in allergic asthma) within 4 hours, the lung effect is typically longer but within 24 hours.  (that is why it is important to start Prednisone early in an asthma attack).

Depends on cause: How quickly Prednisone works , if it works at all, depends on the cause of the joint pain. If you have an inflammatory arthritis like rheumatoid it may start helping within days. If you have severe bone on bone osteoarthritis it may not help at all that may only be helped with an injection into the joint.

Mgt: The steroid effect depends on the underlying problem and extent of the inflammation. The steroids will start to be more effective after a few doses. At day 3, you should experience improvement if you are responding to the medication.

Not the standard Rx : We don't use oral Prednisone to treat the lumbar facet disease. It is a type of osteoarthritis or wear and tear arthritis. Sometimes we can inject the facet joint. Your best treatment is physical therapy also.

Depends: On the hives, prednisone dose, other medications you are on, the cause for the  hives, if any. Kindly see your doctor and consider referral to a dermatologist or an allergist, meanwhile, unless contraindicated, you can take OTC antihistamines, preferably the newer ones, as cetirizine or loratadine, twice a day, which is double the recommended dose, till you see your doctor/specialist, best wishes
