Maybe: A few hours. Can be 4-12 hours, but not all patients with appendicitis get the same feelings. While mid abdominal pain that moves to the right lower abdomen is a "classic" sign, not all people get that symptom. See a doctor if you are concerned.

ive had lower right side stomach pain for 4 1/2 years that comes and goes. its become more frequent with time. ive had a doctor suggest subacute appendicitis. nothing shows on scans. would a surgeon take it out? i cant do this anymore?

Yes: Acute appendicitis usually starts with pain/discomfort around the belly button. So yes, both sides in front and just at waist. Typically within 12 to 24 hours the pain settles into the right lower abdomen and intensifies to the point you want/need to come in. At the er, the docs make the diagnosis by exam, labs, imaging. The treatment is surgical removal of the appendix.

Many things: Other causes of rlq pain would include pid(pelvic inflammatory disease), tubo-ovaarian abscess, meckels diverticulitis, sigmoid colon diverticulitis, various types of colitis, muscular abdominal wall pain, gastroenteritis with diarrhea etc.

Usually a few: Hours, if it is acute appendicitis.  But your pain could be from other sources, such as kidney stones, crohn's disease, bowel cancer.  So see your doctor for more accurate diagnosis.

No: Pain from appendicitis becomes progressively worse over the period of one day. While a dull ache next to the belly button could be the earliest signs of appendicitis, pain from appendicitis will become sharp and intense and usually migrate to the right lower quadrant.

Urgent exam needed: While there are many causes of abdominal pain, sharp localized pain with fever is always worrisome. A medical exam is needed right away. If it is appendicitis, the sooner it can be diagnosed, the better. Delays often lead to major complications.
