Many things: Other causes of rlq pain would include pid(pelvic inflammatory disease), tubo-ovaarian abscess, meckels diverticulitis, sigmoid colon diverticulitis, various types of colitis, muscular abdominal wall pain, gastroenteritis with diarrhea etc.
