Depends: It depends on what is the cause.  If you have eliminated the cause of your thinned vaginal skin and you are producing normal amounts of natural hormones, then you should notice improvement within 2 weeks.  Nevertheless, i would follow the recommendation of the doctor that prescribed estrace, (estradiol) as I do not really have enough information to assess your disease process.

: Discuss with your gyn. If your symptoms are typical, she may be willing to prescribe the right treatment without exam; or perhaps with visual exam without touching you. Maybe you could self collect a vaginal swab if a sample is needed. If even these aren't suitable, work with both your gyn and your PTSD counselor; together you probably can come up with a plan that would work. Good luck.

: As long as you take the dual combination you are safe.At the age of 66 you may be advised to take a drug holiday. WhatI mean discontinue 
the medication for a month and if the symptoms fail to appear or are much less you may continue with no drugs. Good Luck

: Gradual bloodstream levels of estrogen by strictly vaginal use of estrogen sources , and can cause proliferation of endometrial cells.  Long term use may be managed by your prescribing physician by occasional ultrasound imaging of the endometrial lining. If the lining is equal to one cm. in depth, a biopsy may be indicated.

Little Risk: Vagifem are estrace (estradiol) vaginal cream act locally.  Without a Family History of breast cancer, the risk is very little.
You should try to quit smoking to decrease your risk of lung cancer, emphysema, and chronic bronchitis.
I'm not sure what you mean when you say you had a partial hysterectomy.  If you mean you still have a cervix, then you may get some spotting if your cervix has endometrial cells.

: Don't expect much absorption from such areas. Vaginal tissue is a mucous membrane that can absorb this medication while normal skin is designed to block materials from entering the body. Such application would waste the material. TV adds that tout the "deep penetration" of pain creams are a myth. They cause a superficial sensation in the area that distracts the brain, they don't go deep.

Depends . . .: Tricky question to answer bc depends upon definitions. Pelvic/vaginal exams are separate from Pap smears which look for cervical cancer. Assuming total hysterectomy (TH) for non-cancerous reasons, there's no further need for Pap smears. Common misunderstanding that TH includes removing ovaries - this is not true, as it only refers to removing whole uterus. If ovaries removed too then stop pelvic
