Nothing: It's a lay term, not a medical one. There are literally thousands of hormones and "out of whack" in no way describes what the problem is. Hormonal medicine is very intricate and complex, and it can't be simplified into a single statement such as "our of whack". Discuss w/your physician.

PMS: I understand your concern.

Changes in Hormones-Estrogen and is important cause of Pre-menstruation Syndrome.

Your hormones are not out of whack if you have regular periods.
Consider:
Exercise
Eat healthy foods
Avoid salt, sugary foods, caffeine, and alcohol, smoking.
Get enough sleep. 
Find healthy ways to cope with stress

It depends: Serious acne in a woman suggests excess androgens especially if the monthly cycle isn't as it is for most women. Any general physician knows how to do this workup. A man between age 12 and age 96 or so is probably maxed out on testosterone, and whether or not he has acne says nothing about his hornomal milieu -- ask about sexual functioning instead.

May be: It's possible but the more important thing to find out is why your cycle was irregular in the first place.

No : That does not really make sense. If you have had those hormones checked there that is not the problem. If your menstrusl cycles are normal then you female hormones are fine. I'd see you doctor and investigate nonhormonal causes of your symptoms.

Hair loss : Hair loss in a 25 year old is not likely to be related to female hormone levels. Alopecia areata is the most likely. Hypothyroidism is another common cause. If you are overweight elevated testosterone is another.

Yes: In fact thyroid hormone dysfunction is one of the causes of unintentional weight gain for some people.  Also being depressed or having anxiety can lead to weight gain.  In women other hormone imbalance conditions such as pcos also result in weight gain.  For more specific info for you, talk to your doctor.

No: It's definitely possible to miss a pill or two and still have your body feel and be normal.  The Progesterone only pill is less forgiving, but the combined estrogen/progesterone pill is typically going to keep your body at an even state, even if you miss a pill now and then.  If that didn't answer your question, consider rephrasing, best of luck!

Possible bleeding: When you begin taking a birth control pill and then stop your body may "think" that it is time to have your period.  It may take a cycle to begin to have your period at its usual time. But you should not feel ill in general after this. If you do need birth control, however, you should consider trying again as you might have not felt comfortable for another reason

Hormonal imbalance: The only way for you to figure that out is to decide where you think you have hormone issues and where you don't.

When you have that list please discuss that list with your doctor.

Chances are it is not hormones - your doctor will let you know.

If he/she does not find anything wrong a visit to a Psychologist might be in order.
