More information: To accurately answer your question, we would need to know what disease you're talking about.  If it's periodontal disease, the only "home remedy" available is routine and meticulous brushing and flossing along with an antibacterial mouthwash like listerine or crest prohealth.  This only helps after all the bacterial tartar is removed by your dentist, however.

Try reducing contact time and using them less frequently.  Sometimes brushing with a desensitizing  toothpaste like sensodyne prior to whitening helps.

Yes and no!: There are many ways to whiten your teeth. Just look on the internet. Whitening is actually cleaning the enamel or outside layer of your teeth. But be careful! the layer underneath is dentin and that can be very sensitive. Too much whitening can damage this layer, cause extreme discomfort and and actually damage the nerve inside your teeth.

By brushing properly using a soft toothbrush you can get great result. You can also use some over the counter whitening toothpaste. This is more of a maintenance plan and you should have a professional cleaning and whitening done by a dentist.
