Cold sore: The sore is in the process of healing and you should leave it alone.
For good health - Have a diet rich in fresh vegetables, fruits, whole grains, low fat milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex.

Oral Lesions: There are multiple causes for lip lesions based on how theylook and a host of other data including how they appeared for how long etc.  Your medical history including sexual history is important. Some causes although unlikely in your age is cancer like melanoma or infectious causes such as fungus and viruses such as herpes or chicken pox. It could also just be skin irritation. See your Dr. first.

Lip swelling: You could have burn your upper lip when eating hot food like pizza. See an ENT doctor if the swelling recurs.

See your dentist: In order to determine if the lump is an infection, growth, etc?  Your dentist can examine you, take some x-rays and either treat you if it's a dental problem or refer you to a physician if it's not.

Lip lump: If it's an ulcer that has been there a few days than maybe.  But usually cold sores on on the dry part of the lip and aren't lumpy.  There are other things, like mucoceles (come after accidentally biting the lip) or tumors (including cancer) that could be the cause.  Get it checked out if it isn't going away.

Acne comedone: If the dead skin cells in the skin pore get trapped (and mixed with some oils) but don't form a hard lump; when the lump is squeezed, the mixture comes out like a long thing. However, the squeezing may start an inflammatory reaction and cause a pustule or pimple to form the next day or two.
