Depends: If armpit rash is caused by allergic/irritant contact, cortisone might help. If rash is yeast infection or caused by fungus, lotrimin (clotrimazole) would help. You may need to see a dermatologist to get idea what cause it, because wrong medication may make it worse. For example, if the rash is a fungal infection and you use cortisone, it would make it worse and spreading.

Hard to say : These are often fungal triggered by a yeast in poop that takes advantage of the moisture in the area. Unless your cream contained an anti fungal (cortisone is not) it may not improve. An exam may be needed if this persists.

Underlying cause: You should talk to your child's doctor about sources of reinfection: pacifiers, bottle nipples, your breasts, if your son also has oral thrush. If not, just being in wet diapers predisposes a baby to candidal rashes. Make sure you are changing him often. There is also an oral medication that might be appropriate; speak to your child's doctor.

Moisturizer: Apply a moisturizer several times daily. The moisturizer should contain no fragrance. A thin layer of an antibiotic ointment may be used three time daily.  Use gloves while cleaning and please follow up with your PCP.

Locoid (hydrocortisone butyrate) and other topical steroids are good for non-infectious inflammatory disorders of the skin like eczema, dermatitis, psoriasis and allergic contact dermatitis.  However, fungal and bacterial skin infections will do worse with topical steroids.  See a doctor if it does not improve.
