Many muscles: The symptoms may be due to sleeping position, previous arm immobilization, etc. Adhesive capsulitis (frozen shoulder) symptoms begin gradually, worsen over time, &amp; then resolve within 1-2 yrs. Conservative measures (heating pad, stretching) might help. Frozen: Best conservative treatment includes intraarticular cortisone injection and physical therapy. If not improving then will need surgical lysis of adhesions/capsular release and/or manipulation under anesthesia.

Active inflammation: In the initial and second stages of frozen shoulder there is active inflammation and formation of adhesions/scar tissue which increase with pressure on the area or stretching the capsule. Cortisone injection in the sub acromial or supraspinatous helps in some cases, follow by 1 month of tapering oral corticosteroids.

No best answer: The real question is what is the shoulder pain from? Is it inflammation of a bursa in the shoulder, muscle related or even a tendon tear? The possibility of a referred pain into the shoulder from neck joints(facets) or nerve roots is also possible. Finding the underlying problem is the first task. Then making sure your laying in a position that will not worsen that underlying pathology.

Anything Comfortable: You should just listen to your body and sleep in a comfortable position. Your doc and/or physical therapist may have some more specific suggestions depending on the specifics of your injury but comfort is a good guide.  (most will find laying on their back with their arm resting on a pillow.  Some, who cannot sleep on their back, find using a recliner and pillow to work well.).

Shoulder impingement: Causes of shoulder impingement are usually overuse, shoulder labrum or rotator cuff tears, and certain anatomic configurations of the shoulder joint. I wouldn't ascribe impingement syndrome to sleeping patterns or poor mattress.

Muscular: Most back pain in this area is muscular.  Your triggers also sound muscular.  If working out is aggravating it, you may need to eval your technique.  A trial of physical therapy may help to re-educate the muscles in this are and work on any imbalances.

Unknown: We do not know the cause of a frozen shoulder. We know that it is a painful syndrome that limits shoulder motion. Most of the time it will resolve on it's own over five years. Injections and physical therapy will often help the problem resolve over 3-6 months.

See ortho doc: Frozen shoulder, aka adhesive capsulitis, is an inflammatory condition condition causing pain and shoulder stiffness. Treatment should be directed at inflammation 1st, with nsaid's or cortisone injection, and then pt for the stiffness. This should be done under direction of your ortho doc.
